Otoplasty is a great procedure to reshape prominent ears.  There are three regions that may contribute to ear prominence.  In the upper 1/3 of the ear, a patient might lack congenital folds in the cartilage.  In the middle 1/3 of the ear, the concha (bowl of the ear) might be enlarged.  In the lower 1/3 of the ear, the cartilage and ear lobe might be excessive.
